In our hospital (British Columbia, Canada)  Vitamin K is given on admission to the post partum unit and if the delivery is in the single room maternity care unit, it is to be given within 6 hours of birth.  I often gave it when the mom was up out of bed and in the shower or around the 2 hour mark.  I also gave it while baby was nursing, it just depended on how things were flowing.  Erythromycin eye ointment is required by law here and it is to be given within an hour of birth, though I frequently gave it later.  It can be refused by parents, they have to sign a waiver if they want to refuse it.  Both of these procedures can easily be done with baby skin to skin with the mother, as can most other routine things, like vital sign checks of mom and baby.
