Hello lactnetters,

I've had an interesting case presented to me recently, of which I've not seen before, and the mother has asked me to post for any information the global lactation community may have. She is currently 36/40 pregnant, has had 2 previous miscarriages, and has been given a diagnosis of cystic ovaries when she was 16 years old. This mother was commenced on a natural progesterone cream for 8 months before she fell pregnant (not clear what sort of HP prescribed that).

Any way, she has been diagnosed with a R breast 'ductasia' for the past 7-8 years. She has had a few ultrasounds showing several blocked ducts and a benign lump behind the ducts. A fine needle biopsy was NAD . She has always been able "to squeeze out some yellowy browny grey discharge" for the whole time she has had this. The discharge was tested a couple of years ago and mother states "there were no cancer cells" but is unsure about microbiology. She has never been put on antibiotics.

This mother is very keen to breastfeed, but would like to know a few things, - will the blockages be a problem? will the discharge be a problem?

She realises that it will be a wait and see what happens scenario. Is it possible that a lactating breast, with all the properties of breast milk, actually improve/cure this situation?

We have had lots of antenatal discussion on positioning and attachment etc to optimise the feeding from birth. She has noticed breast changes, but no increase in discharge or any pain since pregnancy.

The mother has been offered laser treatment, but she is reluctant to do this as she doesn't want to impact on her feeding potential.

Has anyone seen this before, had experience or can offer some words of advice please.

Thank you in advance
