We use CPN.  We have a maternal history section, a maternal breast assessment, an infant assessment section and a breastfeeding evaluation section.  We document lactation interventions, latch score, Breastfeeding Risk of Abandonment score, Acuity Level, equipment used, education done, hand-outs given, etc.  IT's very detailed and works great!
