just to add my two cents here, i encourage my clients to think of it as "appetizer and dinner" on one side, and if the baby needs "dessert" (or mom needs baby to take dessert for comfort) then to switch to the second breast.  i feel this helps encourage them to pay attention to their babies and their bodies rather then watching the clock.  i also give them the example that we don't eat to a clock, we eat until we are full and then move on, i have found that while most of my parents want to schedule their babies this language helps to shift their focus to feeding rather then scheduling.
