Small, frequent stools can be a sign of food sensitivities - are these
small? That coupled with the diaper rash make me think along those lines.
Any other allergic symptoms? Any known food sensitivities? Eczema?
Frequent night-waking? Reflux? Cradle cap? Baby acne?
Did mom or baby have antibiotics at the time of birth, or either one since?
