Lisa,
I think this was discussed recently and I think Cathy Genna responded and
suggested to center the nipple at latch so it doesn't immediately touch the
palate and elicit gag. The other possibility is the use of a nipple shield
if the nipple is pliable enough and can be comfortable inside the shield.

I have a post partum mom 2 days out, attempting to breast feed but her
nipples are very long, probably the length of a thumb,I'm guessing. The baby is
trying but gags or pulls back only wanting to take in the end of the
nipple. Any suggestions? I am afraid that the baby will be unable to take as
much tissue as needed and cause discomfort for mom, pus not get the volume
needed.
