Anne writes about relactation with a 5 mos old baby and I'd suggest she  
begin by breastfeeding with lactation aid, preferably the Lact Aid as it is 
less  obtrusive and in the longer run what moms who use one long term like 
best;  slower flow, too.  If baby will take her breast and feed with 
supplementer,  no need for shield but if baby balks at going near breast then a shield 
would be  fine.  I worked with an adoptive mom who first breastfed her 4.5 
mos  old in my group at my suggestion and her baby went to breast as if 
she'd always  been there.  Oh she had sore nipples and had to tweak the latch 
but it was  a beautiful experience and this mom went on to breastfeed for over 
a yr.   She never did get much milk, despite use of domperidone and herbs 
but it was the  relationship that mattered; I will always think it was 
because she had a close  relationship with her baby to begin with that nursing her 
came so readily.   (She went on to nurse her second adopted child from the 
beginning for two yr I  think, and this time used donor milk at breast).  
 
As far as increasing milk, I'd want to know what mom wants, to have her  
baby at breast mostly or to give her baby her milk.  If the former,  
establishing the relationship of breastfeeding most important and the milk that  
would likely ensue with breastfeeding would be a lovely bit of gravy, so to  
speak.  If milk is her main objective, I'd encourage the Goldfarb protocol  and 
use of domperidone and goat's rue and perhaps other galactagogues.   Lisa 
Marasco and Diana's West's book, Making More Milk, would be an excellent  
resource for you, mom and midwife.  Good luck to this mom who I would tend  to 
encourage to focus on nursing her baby and let the relationship be first and 
 foremost so it can be savored as it should be.
