Arly asks what actual practice in this situation is, after discovering that the 1999 edition of Lawrence advises that mothers should express and discard their milk until 24 hours into antibiotic treatment. Arly asks, very sensibly, whether this confers any real protection since the baby will undoubtedly already have been exposed by the time the mother is diagnosed.
I've never ever heard of such a recommendation and can say unequivocally that practice here is to continue breastfeeding right through the maternal infection. I don't have Lawrence so I looked up infectious disease in the mother in the BF Triage Tool, 2005 edition, and read that 'Breastfeeding is safe for mothers who are Group B strep positive. Mother should be treated by her provider.' The reference given for this advice is the 26th edition of the Red Book, from the AAP Committee on Infectious Disease, in 2003.
I'd be curious to know what later editions of Lawrence say, if anyone cares to look it up.
