Subject: therapy after frenotomy



Hi, I posted on Mar. 3rd about a baby who had a posterior tie clipped and now her tongue is discoordinated. Thanks for the replies, Liz, Kathy,and Jane. It has been over a month now and not much improvement- I have reviewed 'Supporting Sucking Skills' and see somehints about exercises there, but I guess I still don't feel quite confident about which ones to suggest. I guess a little gentle pacifier tug of war and some stretching wouldn't be harmful, but after that thinkI'll refer her to the early intervention program. What the Mom is really wondering, tho, is if she can ignore this til baby's older. I suspect that it's better to deal with it earlier, but wondering what people with more experience would say... Thanks so much, Mary Taylor, IBCLC  How long ago was the clipping? With posterior tongue ties, we often see

good results immediately, then the baby disorganizes (I imagine the

tongue gets tired, as it has not had it's full range of motion and

making the right movements is a little difficult, like walking when

you've had a cast removed from your leg). By 5-7 days the baby usually

recovers and is sucking much better. If this is not so, then we start

exercises to help.

~~~ Wow, I wonder how this compares to the incidence of positive serology  

in the general population. Good information to know. Women can be caught 

between  giving their babies the substandard but theoretically  standardized 

liquid that is AIM and giving their babies  live human  milk from a loving 

friend or a concerned neighbor with unknown status.

I'm wondering about the 3.3%.  It seems like a small number to me, compared

to things like a doubling of risk for diabetes faced by infants who don't

grow on human milk.  Is 3.3% really considered 'significant' in the world of

research?

Main outcome measures Results of serological screening tests performed on

potential donors. Results Of 1091 potential donors, 3.3% were positive on

screening serology, including 6 syphilis, 17 hepatitis B, 3 hepatitis C, 6

HTLV and 4 HIV.

I agree Ingrid.

And in all retrospect, if moms want to get milk privately, they have every right to ask the other mom to be tested and if there is further concern, they can pasteurize at home.

It's still better than giving formula if donor mom is free and clear and milk pasteurized.

What kind of freezer is she attemtping to freeze her milk in and what is the temp on that freezer?

I have been milk banking for quite some time now and I have NEVER heard of this or seen this with our millions of ounces of hundreds of donors that have come through our bank.

The breastmilk that won't freeze  is in our very nice state-of-the-art commercial freezer in out NICN unit. If you do an advanced search on Lactnet using freeze as your key word you willl find 16 posts about this subject. It obviously has happened to a number of other interested persons. However, there are no explanations. Would you concur that we can use the milk as usual?

Subject: BYO formula?



In practice, at BFHI hospitals, do parents bring in their own artificial feeds if they want to feed it to their babies, but it's not medically neccessary?

What interaction happens when parents want to both bf and substitute at the same time, but staff is teaching exclusivity? Is supplementation less frequent or non exsistant because it's not handy and free of charge to the parent?

Subject: breastmilk leftovers to baby's sibling?



I have a 5 month old baby who has been strictly breastfed. I am overproducing and have alot (30 bottles) in the freezer. Are there benefits to giving my extra milk to my older child? He is 4 and suffers from eczema and cow's milk allergy (which only started after he was weaned at 1 yr old). He has some behavioral challenges and I can't help but wonder if it might help him. Wondered what you all think.  I know milk banks are great too...but just wondered.

Exactly WHAT is happening with the suck that's not normal? The exercises 

we choose are determined by what the baby needs to learn or unlearn!



If the baby's tongue movements are uncoordinated, this might be more 

than tt, there might be neuro issues. Sometimes "walking back on the 

tongue" and the "slideback" (slide fingertip along tongue midline from 

front to about mid tongue) help, as they mimic the normal sequence of 

muscle activation for the swallow.
