Hi Jessica,
This could be vasospasms.  Ask the mother to describe what her nipples look
like when she feels the pain.  Are they blanching?  Ask about a history of
renauds as well.  I have had renauds most of my life and though my nipples
blanch after a feed or whenever they are cold they never hurt but I have
experience deep breast pain.  Perhaps her nipples are being pinched during
the feed and that is causing the vasospasm.  Again, look at her nipples
after the feed or have her describe them to you.  She may just need to warm
her nipples after a feed or fix the latch.
