There is such a specialty "Mammalogist" in Russia. I was introduced to a doctor-mammalogist in Moscow. She specialized in mammalogy during her residency(MD) and has PhD as well ( her theme was Reconstruction of Milk Ducts after Breast Cancer). She also took a local course for Lactation Consultants. She noted that she always will assess the latch when she sees a patient with mastitis. She told me that she can hand express any blocked duct. I saw her expressing, it was unbelievable! I really hope that she will be studying and taking IBCLC exam in the nearest future. By the way, it is very common for many doctors in Russia to prescribe Bromocriptine as a treatment for mastitis. And the rate of mastitis is very high in the countries I visited.
