How did the mother become pregnant?  Odds are that a 48yr old mother
pregnant with her first has had fertility treatments.  Many of the hormonal
concerns that cause infertility will also interfere with lactation....
Have you further explored that she did not notice breast changes during
pregnancy?
Galactagogues are not the solution if the mother needs medical workup...
