Have you tried pre-filling the nipple shield with expressed milk? Mom
can either compress her breast if that won't push the shield off, or she
can use a curved tip syringe to place the milk inside from through the
holes in the tip of the teat... There's a photo of this in Selecting and
Using Breastfeeding Tools (Hale) as well as photos of Linda Pohl's
method of putting a nipple shield on so the milk doesn't drip right out.
The idea is to give baby an instant reward, and to trigger a swallow so
that it starts up the suck-swallow-breathe cycle.
I'd also make sure the baby can breathe well. Sometimes babies with
blocked noses (mucous, choanal atresia, etc) act this way.
