Although the fibrocystic portion is interesting it seems most likely that the letdown issue is related to the tissue breakdown of the nipple, so it seems important to address 1. The pumping issue in depth-perhaps larger flanges in addition to the olive oil 2. The baby's oral issues and 3. the output for mom. In addition to continued use of relaxation, visualization, and skin to skin (even though she hasn't yet seen the effects) implementing regular compression and hands on pumping should yield quite a bit more per pumping even in the absence of an apparent letdown thus far.
