After the recent posts on this, I decided to visit the Mirena web site. I
was rather surprised in how the Patient Information pdf differed from the
pdf available under Physician Information when I did a search of each re:
breastfeeding  (their spelling breast-feeding). Quite a different tone...

If someone was set on a progestin-only option, would think pills would be
the starting place. Can't miss even a day, but at least a pill is easier to
stop if production is affected. Post-IUD insertion cramping can be
uncomfortable so mothers think twice about its removal, implants are not
exactly easy to remove -- or get an appt for that purpose, and there's no
going back with injectables.

Perhaps the biggest problem, though, is that so many health providers do not
believe production is affected for some mothers. Therefore, they don't
provide them with the caveat to watch for changes in production. I've had
mothers call who've experienced a fairly major dip within 2 weeks of
introducing a progestin-only method. Most were encouraged by docs that this
was a "safe" option during lactation, so they waited to call until
production kept going lower and lower. Said they sort of wondered if the
contraceptive may be related but their docs had assured them it wouldn't
cause a problem. Sometimes there are other confounders, e.g. mom had a minor
illness or returned to work about the same time -- possibly a little
before/after. I wonder if it's a combo of factors?
