Hi Anne,
I would be considering treating this mother systemically for bacterial
infection such as staph (actually I wouldn't be treating, but referring to a
primary care provider). I would also see if mother/baby could adapt to some
feeds with lactation aid and decrease the number of pumpings mother has to
do. I think mother and baby need that contact. I would ask mother to
continue hand expressing after feeds, even 5 minutes would help.
I would not be reluctant to suggest she consider galactagogues unless I
thought she had oversupply.
