In an effort to stem the flood of formula being given to breastfed 
babies in the hospital, some hospitals place formula in their 
medication distribution system such as Pyxis. This will log each bottle 
used, the name of the nurse who withdrew it, and the reason for its 
use. This helps educate nurses regarding overuse or inappropriate use 
of formula and keeps them from loading up mothers with a bag of bottles 
at discharge. Some hospitals that require parents to sign consent forms 
for formula supplementation have policies that require a lactation 
consultant to observe the next breastfeeding following bottle 
supplementation. Some consent forms are very simple and others list 
possible side effects of formula supplementation.
