Hi Lisa

Have you considered Pagets Disease of the nipple, which is a form of cancer? 
You can google a fair bit of information on it.  Would be worth checking 
out, even though it's very rare in a young woman.  Time for that doctor's 
appointment, I reckon.  Hopefully it's just eczema or an infection, but best 
to know sooner rather than later.
