I'd love to see what happens in the baby's mouth on ultrasound before
blaming it all on maternal hypersensitivity. We've seen all kinds of
distortions in our study, including a baby who bent the mom's nipple to
a 90 degree angle during sucking! The big red flag here is that there is
more pain on the initial latch, which usually means the grasp is not
quite right, even if it looks okay from the outside.
The other thing I'd look at is if there is a chronic low-grade infection
of the nipple that could be ramping up the pain.
