What's the alternative for documentation? We have the latch score and/or minutes. We encourage latch score as that's more evidence-based. Nurse charts observed or reported.

Subject: LATCH documentation
Hello all. I am interested in knowing how often it's required that nurses document on the LATCH scale for those of you in a hospital setting. References for your practice or policy would be great!
Thanks so much!
