Hello,

I have permission from mom to post (she is waiting to hear other suggestions).  Mom is at the end of her rope - is starting to pump and bottle from the pain.  Saw mom a few days pp to help with latch - had a tongue that needed clipping (as did her first - took weeks the first time, got it done in a few days this time).  I did a follow up to help latch a bit more now that baby had better tongue function.  Felt much better (no nipple damage seen, just some redness/soreness).  

Saw her a couple of weeks later - had bad thrush first time around and nipples were still sore.  Only seeing slight nipple tip pinching at end of feed - felt most pain was nerves (both psychological and since it was so bad last time nerves a bit oversensitive).  On right nipple there is a slight pinching on bottom (corner in relation to baby's mouth during cross cradle hold) and on both 'corners' on left nipple.  Left nipple looks 'pointy' after a feed and right has a half moon shape.  (A bit hard to describe - have been doing only phone consultations since, a few weeks ago baby wasn't latching to both breasts , tried the slide over technique and that seemed to work.)

She also says that left nipple that bottom 'corner' that is pinched is 'popped out' a bit all the time.  I did suggest if it doesn't seem to get better after a week or so of pumping to get it checked out, that it may actually be some damage baby has done from the bad latch.

I figured that once baby got a bit bigger opening the mouth wider should be a bit easier - she has had some body work done after the clipping but he jaw is still a bit tight, even now that he is 11 weeks old it still feels like he can't get on deep enough.  Although some may be habit - when she tried to latch him deeper he goes back to his regular latch and fussies during the whole feeding.

Any ideas of what else might be going on?  If you need more info I can get it from mom (or my notes).  He is growing and developing great - she has plenty of milk and he has no trouble getting enough to eat - he just hurts mom during the feeding.

Thanks for any ideas you guys can have for this mom.
