1) Salt water soaks:
Baby issues: It would be important to rinse off the sodium chloride (NaCl -
common salt) with clear water before the baby next attaches to the breast,
to avoid NaCl consumption by the baby. I guess rinsing would also settle
the iodine question as regards the baby. (Anyone have a definitive answer?)
Maternal issues: I do not know if iodine in iodised salt, in the solution
used, would be significantly absorbed through the mother's skin. Research
in Australia has shown that a large proportion of women of childbearing age
are deficient in iodine. So absorption of small amounts would seem to be a
theoretical advantage. There was a paper on iodine deficiency in Australia
by Professor Eastman at a conference of the New South Wales Lactation
College two years ago.
2) Ice on nipples:
I used to use it for slightly numbing the nipple (giving the mother more
confidence for latching), and the baby's mouth would quickly warm the nipple
and restore normal sensation. However, the main obstacle is whether the
mother has a history of Raynaud's-like nipple vasospasm, in which case
applying cold could create excruciating pain. There are usually better ways
than using ice.
