Could her skin be reacting to disposable nursing pads?  If she is using
disposable pads, the chemicals in them may cause tissue damage when they are
damp and against her nipple skin, just like some brands of disposable
diapers cause diaper rash for some babies and not for others.

Have her try plain, white, cotton men's handkerchiefs, folded into squares,
as breast pads.  They are available in the men's department of major
stores.  They come several to a package and are not very expensive.  When
you open them up, they are a single fabric layer, that can be hand-washed in
the bathroom at every feeding and hung to dry quickly, before the next
feeding.  100% Cotton is not likely to react with sensitive skin.

This has been very effective for some moms I have worked with.
