Familial lipoprotein lipase deficiency needs to be considered. 

I would also suggest she consider getting away from the paediatrician
who has already shown himself to be irrationally breastfeeding-hostile,
and seeking a second opinion, perhaps from a pediatric endocrinologist.

If the child does end up needing to be on a lower fat diet after further
investigation, is the mother open to pumping and skimming at least some
of the feeds?
