I have seen this. Not with inverted nipples, though.
It seems these kids are applying abnormally high negative pressure, and suck
the nipple skin through the little openings.
I would not use the shield anymore, make sure she is pumping with large
enough flanges to stretch out the tissue, and keep trying. Lots of time
with truly inverted nipples they need large flanges, and the baby may have
to grow into being able to reach past the nipple tissue onto the breast.
