Was this baby ever evaluated for posterior TT? Also, when I see similar problems in babies born of
the same mom, I remember that both babies grew in the same uterus. Often, the shape of mom's uterus
or the structure of her pelvis means her babies are similarly restricted in utero. Those babies very often
need CST in spite of a hospital-"normal" birth or even a homebirth.

Did you try a nipple shield? With the tube under neath it if needed?
"I tried the baby with a FF tube with some of her pumped milk to see if I
could elicit a good rhythmic suck. The baby did beautifully once the tube
was inserted. Long draws, good jaw movement, etc. The minute the tube was
removed, he stopped."
So, no stimulus to suckle w/o the tube? What is the shape of the palate? Does baby draw a finger in when
offered?
"I left her with instructions to feed with the tube, use as much breast milk
as she could get, formula as needed, and of course pumping. Now it is 3
days later. Baby still will do nothing without the tube. Mom is afraid he
is getting addicted to it, and will never nurse without it. On the other
hand, he is still not transferring milk without it."
Again either TT or restriction creating an inability to extend come to mind. CST.
"On her discharge papers the doc wrote minor head lag. I felt that his neck
muscles were quite weak."
CST (my preference is always a ped chiropractor--do not know who you have there--PT who does CST,
DO?)--should have been immediate.
