Hi Mary,  I am only addressing the meconium portion of your statement as i 
must run to a meeting.  What he must have said was Meconium getting below 
the cords.  (vocal).  Has nothing to do the the umbilical cord.  Babies swallow 
and breathe the amniotic fluid.  Unfortunately, when they take a deep breathe 
following birth, the mec gets  stuck to the alveoli in the lungs.  That is why he 
cut the cord quickly, hoping to get the baby suctioned before a big cry.  We 
used to be gentle handling a baby until we could get it to warmer and suction 
out the mec.  It can really be a dangerous situation.  I had a friend of mine 
who's baby was born in a level III hospital, mec stained fluid, started crying 
before NICU RN's were able to suction below cords.  Baby on ventilator, later 
passed away.  Meconium is one of those unpredictable factors that can 
happpen at any birth.  My point is I think she may have misunderstood the 
resident at that moment, very understandable, lots of things happening fast.
I will read your post indepth later.  Sorry for only replying to one of your 
questions.  I'm sure one of our lactnetters can respond to the rest of your 
questions.
