Unless things have totally changed in the past few years, I know IBLCE is employing an outside source to proctor the exams, special needs should not be a problem.
We have nursing moms, people who need to go to use the restroom, people who have other needs, etc - all of the time. Has not been an issue with us in Israel. All we needed was prior notice.
Has anyone really check with IBLCE about all of this. Breastfeeding babies should not be an issue during any professional exam, anywhere at anytime.
