Great post.  I have not been handing out those privacy forms.  I assume I am 
covered by the hospital (yes I am now that I think about it) but what about 
private practice?  You've made my wheels go round.  I've got to think about 
this one.  Any private practice providers want to share what you have been 
doing,regarding HIPPA and this particular piece of paper that Liz is describing.  
Is their any difference for a RN, IBCLC or IBCLC.  I'm really curious what you 
private practice folks do.
