Nikki, I've had this type of situation happen also.  It would never have been 
caught had I not done a home visit.  One of the reasons why only doing 
assessments and crisis management in a clinic setting does not give the LC 
the whole picture.

Glad you were able to figure it out, AND glad that the mother was able to lose 
the pillow.
