The physical aspects of this case may be challenging but do not seem to be
the real issue here. Does this woman want to breastfeed? Not engaging in the
feed when the baby is trying to latch may be the mother's way of saying
again that she does not want to do this. She already expressed this before
the birth. Would she be willing to pump and bottle feed? Some mother's with
really good supplies and large breast storage capacity can pump less often
and still get a good amount of milk once the supply is established. I had a
mother who pumped 3x per day at 3 months and could provide all the milk she
needed. (Her baby latched for the first time at 3 1/2 months and went on to
breastfeed for 2 years. Another story.). In the case described, the mother
may be having issues of PPD or issues handling her breasts which are large,
flat and pendulous. Having a frank, supportive conversation with some open
ended questions may help. Let this mother know that you will support her
discision and help her, even if it means weaning. She needs to know that she
has some control of how she will ultimatly feed her baby. These situations
