Ideas for this scenerio:

Large or long nipples, and/or deep ducts (Does mom need large pump flanges
to remove milk?  Can be a clue)

Submucosal cleft palate or posterior tongue-tie.

Baby too underweight to be strong enough to do well. This is actually fairly
likely if no BM for 2 days at about 2 weeks of age with continued LOSS.
Would push supplement hard to get weight up ASAP and may take care of it.

Of course, then there are the zebras...like metabolic diseases.
