I work in a hospital and each item you mentioned are reasons babies get 
supplemented.
Are you looking for the NUMBER one reason at our hospital?

1. Maternal anxiety that baby is not getting enough at breast.
   unrealistic expectations about breastfeeding.  Like, my baby is constantly at 
my breast.  If he/she were getting something they would sleep for (blank) 
hours, so they must be starving.  I have to get some sleep. Put in nursery and 
give bottle if hungry before I get 4 or 5 hours of sleep.  I always gently 
explain that babies are born with an instinct to suckle.  Its survival for them.  
They don't know that it is 2 am, they are bringing in the milk.  Doing what 
nature intended.

2.  Hyperbilirubinemia

3.  Gestational Diabetes
