I'd probably ask the mother why she chose to BF her first child. I'd think something might be mentioned that she believe each of her children deserved. I'd probably also want to explore why she thinks her son would have the response she anticipates. Then I might suggest a number of options -- BF in the hospital, BF when son wasn't around, BF with son around but monitor his response, etc.
What I wouldn't want to do is trivialize her beliefs/feelings simply because she's in a different place than I now am...
