In terms of the question of mothers who don't release to the pump, I think the main reason
is that the ducts may be placed deeper in the breast than for other mothers and need
compression rather than just suction for a good release. Breast massage while pumping
often helps --- I now have moms try all sorts of methods e.g. finger tips, thumb tips and
knuckle rolls to see which works best for them and I find it highly variable mom to mom.
Changing the shield SHAPE can help. The typical shields are entirely conical --- there is an
angled shield that has three grooves on top and a grippy area underneath. These seem to
grab and compress in more areas and usually work better for women with more elastic
breasts. The kind of breasts that just don't really fit into those conical shields. Then there
is all the psychological tricks moms can use to relax. The only one that worked for me
was computer games because it took my mind off work (which was stressful) and milk
production (which I also found stressful). And some women just are better off hand
expressing.
