Jaye,
You're talking about a submucosal frenulum. The lingual frenulum is 
hidden behind a thin fold of the mucous membrane of the floor of the 
mouth. It's easy to snip them, the doc just stretches the tongue with a 
grooved director, and snips through both the mucous membrane and the 
frenulum in one or more snips. Doc can use a topical anesthetic (like 
benzocaine gel). There is more bleeding when the oral mucosa needs to be 
snipped. Dr. Coryllos uses 2 minutes of direct pressure with sterile 
gauze to the site to stop the bleeding, then the baby goes to breast.
