I'm sorry, I should have replied to this sooner. And I'm not sure if this anecdotal evidence will be any help to you or the mother.
I suffered from one sided-nipple eczema when my first child was about that age, 18 months. It was quite painful and upsetting.
After a quick trip to the general practitioner's office, I had in hand a prescription for a general steroid and some very good advice (in my opinion anyway). Always, always, always make sure the baby/toddler does not have any food in his mouth or on his face, before he is put to the breast.
I am red-haired with sensitive skin so it doesn't take much for me to break out in patches of eczema. But after a short round of the steroids, and my constant vigilance to avoid any type of irritant (via food remnants) on my nipples, I have never experienced a case of nipple eczema again.

Update on the mom with one sided nipple/areola "eczema" that has now been
going on 10wks. The APNO (Newman) didn't help, the family dr. referred her to
surgeon. The surgeon's plan is to treat as eczema with a higher potent steroid
Halog (Halcinonide) for 7-10 days and see what happens then if no
improvement try a good yeast medicine (didn't say which medicine) and if that doesn't
work (she said the dr. really doesn't think it is yeast though and that is why
she is trying the steroid first) then a punch biopsy if it does not clear
completely. She didn't want to do the punch biopsy first since she is young at
35 and no risk factors for cancer and felt like a mammogram would not be
helpful right now since she felt no lump and mom is young and lactating.
I do have Hale's 2006 which all it says about this drug is "Do not use on
nipple. Some caution is
recommended if used on large areas on the body." The only thing the dr. said
was not to nurse
on that side. Mom says she is a horrible pumper and worries about finding
the time to be able to pump often with 5 kids and is also worried about getting
a plugged duct or infection.
So my question is, can she nurse around using the medication? She is
supposed to use twice a day so maybe she could apply early morning and late
afternoon and then wipe off so she can nurse at evening/night/through night. Mom felt
it would be harder for her not to nurse at night because during the day she
can distract by offering food/drink and going places (she says baby is less
likely to ask for it out in public) and keeping her busy. The baby is 17mths
and weighs 20 lbs. The baby nurses a lot and still gets about 50% of her
nutrition from mom.
I am awaiting a reply from a breastfeeding friendly family dr. on this drug
but welcome other opinions.
