Sorry to revisit this topic again...

I'm a student midwife in Ontario with a client whose 5 week old baby has a 
posterior tongue tie. It has been clipped once already (about 2-3 weeks ago), 
but babe still has a shallow latch (she falls off frequently) and breastfeeding is 
still painful for mom (despite the thrush she's currently dealing with, which is 
improving quickly). Tongue is still heart-shaped and comes to the gumline. She 
was told at the time of the initial clipping that a second would likely be 
necessary and she was referred to Dr. Coryllos, but that is several hours drive 
for her and her family (she has two little ones under 2, both of whom are 
breastfeeding...babe exclusively...mom is amazing and highly committed to 
breastfeeding). 

She really wants the tongue clipped again but is having a hard time finding 
someone closer who can and will do it.  Any suggestions out there? We're in 
the Eastern Ontario region (Toronto, Ottawa or Montreal would be do-able). 

Also, is there an opinion on whether a nipple shield would be appropriate in 
this case?
