Yes it is common practice to get them to nipple from a bottle before BFing.
I have seen it in Seattle and in Colorado.  It is frustrating because the
videos we show and pamplets we give parents for the special care nursery
state that many babies can BF before the bottle.  I have found it harder to
nipple first then BF because the baby must learn to protect its airway from
the fast flow of an artificial nipple.  Recently I had 34wk twins that were
a great size (both over 5lbs) and they both BF beautifully from the get go.
I did have to do SNS with them or top them of because the NNPs are so
numbers oriented.  These babies just had to get a certain ml/kg/day from day
one.  When I returned a couple days later both had feeding tubes and were
alternating bottle and gavage feeds and only allowed 1 BF "attepmt" a day.
All BF attempts were limited to 15min as to "not tire the babies".  I don't
understand this...I really think it is a matter of hard numbers.  The
medical society has a hard time in trusting nature.   I understand that
premies can feed great at first and then "poop out" as we say, but why go to
the bottle when they could BF from the get go?  These were mom's 4th and 5th
kids and she BF all of the siblings so her milk came in fast, she was good
at it.
