I wonder if some of you could help me with this mom. Permission to post:
35-ish first time mom, very healthy weight and eating habits. Baby born at
term, 5# 12 oz. Mom has small breasts but they are  rounded, symmetrical and
the space between them is not wide, nor are they tubular. Birth vaginal,
labor not over long, no meds or interventions. Mom has had no breast
surgery, on no meds, never had a chronic illness. Health has always been
good. No thyroid probs and no fertility probs. 
Lactogenesis II just did not seem to come. I saw her on day 5 and she had
just a little increase in amount (pumped 20 cc), it looked like colostrum.
It has increased a little but never to full supply and she has had to
supplement with formula. She is pumping after every feeding (Q 2-3 hours)
and feeding the EBM too, of course. She is now on fenugreek but still
struggling. We fixed baby's latch, but besides that, milk just never came in
sufficiently. She is not showing signs of retained placenta and has talked
to her OB about it, too. 
This mom has done everything right and I'd like to be able to tell her why
her body is not producing enough milk for her baby....any ideas on that?
She is considering reglan. I just wonder if I could be missing something.
Could it be that not enough ducts are present even tho she doesn't have
signs of hypoplasia?
