My 3rd baby was born with a high palate, recognised on day 3 by IBLCE from 
my Birth Unit in UK. Characterised by non-nutritive suckling, flutter sucks and 
sore nipples (I had sucessfully breast fed 2 older siblings 15 and 19 years 
before). My enginer husband adapted a thin silcone nipple shield by cutting out 
the lower half with a craft knife to give a shield that fitted partially over the 
top half of the nipple directing my nipple over his tongue to the back of the 
mouth instead of up into the arched palate.
I used breast compression, expressed milk and gave it by cup after each feed 
to offset the effects of reduced milk transfer with the shield. By 12 weeks he 
was able to feed without the shield. Weight gain had been only just ok but 
rose well after shield was stopped. He was breast fed exclusively for 6 months, 
partially for 3 and half years and in tandem with his younger brother fom 
18months of age. My younger son continued to 2 and a half years and self 
weaned, by which time I was close to 47 years and my active breastfeeding 
times were over!
