Took care of this couplet in hospital for 4 days everyday. Latch was "ok".
Baby did open wide. Has a mild midline short frenulum. Fast forward to 4
weeks 2days old. Home visit. Gained 4 lbs. Mom is still sore. No signs of
yeast on mom or baby. When baby comes to breast, he does this open/shut,
open/shut mouth movements. Never a nice wide root. He gets a bottle of
EBM from Grandma at 4am. Talked to them about not pushing bottle thru lips,
but waiting for baby to open. When he yawns he can certainly open wide
then. Nipples look ok but show signs of old cracks that have healed. Using
APNO, now. Instructed mom on side lying position. Baby can self latch and
when they get a deep latch pain goes from "7" to about 4 or 5. I am noticing
that his tongue just stays at the roof of his mouth. I have advised mom to
not let him slurp his way onto the breast. I think he has been getting away
with this since he left the hospital. The latch was much better then? Has he
just gotten into a bad habit? I have asked mom to wait for a nice gape. I'm
sure it will take a little while to re-learn this instinct. How do we get him to
bring his tongue down. His palate is on the flatter side. They don't want to
clip his frenulum just yet. We are giving it a few days to settle the sore
nipples down, pumping every other feed. Using the bottle to elicit a good root
effort and then placing the nipple in his mouth. He is appropriate in all other
aspects. He seems complacent to barely open. He transferred 30cc in about
5 min of have hearted nursing. :) I hope I have given enough info. I am
stumped about his tongue position. Any ideas welcomed.
