Hi Anne,
Had similar situation.  We used cool ice packs made with diapers like you put 
on moms perineum after labor.  If you haven't used this before, pull open part 
of the lining of the back part of the diaper, put the crushed ice inside and 
shake it down into the crotch part of the diaper.  then use the tape tabs to 
seal the ice inside.  apply to breasts.  Its just the right temperature.  Not too 
cold.  This will bring the swelling right down....Since she may be at risk for 
some engorgement problems (since baby is not latching well yet) keep this in 
mind if you start to see her getting to that point.  I also use cabbage and 
keep some in the staff fridge.  I bet in another day or so that baby will 
manage to open  up his mouth and get the nipples and bottom of aerola in no 
problem.  I also encourage moms to sidelying position.  You would be surprised 
how those babies just lunge for the nipple and self latch.  I ALWAYS USE THIS 
WHEN A BABY IS FUSSY AT THE BREAST AND EVERYONE IS WORRIED ABOUT 
GETTING MILK IN THE BABY.  It honestly works 99% of the time.  get a pillow 
behind moms back and make her nice and comfy.  I put them skin to skin and 
tuck a baby blanket under baby's side back and side and then around mom.  
This holds baby in place, and the baby's seem to just relax.  You can see their 
little clenched fists relax.  whole body language changes.  Forgive me if you 
do this already...I just find it so helpful, in our stressful situations.  tuck that 
chin in as far as you can.  If baby can look into moms eyes, you've got good 
positioning.  This works so well when moms have large breasts and their 
tummies are still swollen.  Something about being skin to skin from their head 
to toes along moms tummy.  Good luck to you honey.... Not an easy task we 
do.
