Personal experience: 
My grandson Itai was born at 36 weeks, 2k800 g, emergency CS.  His
glucose was 42, at 1 1/2 hours after birth.  This was considered reason
to give artificial milk, but I refused.  Instead I went to the recovery
room where my daughter Maya was still lying, easily expressed 1 cc of
colostrum and gave it to Itai.  His glucose jumped to 48.  
I have discussed this issue many times with the head Ped, sent him the
ABM protocols, and all to no avail.  If this were any other baby, he
would have gotten artificial milk for sure.
