General anesthesia is only needed if a transverse-vertical frenuloplasty
is being performed - where the floor of the mouth is cut across, and
sewn up and down to provide more tongue elevation. I've only had a few
babies in my practice require this procedure (and the outcomes were very
good).
Dr. Coryllos' minimally invasive office procedure for posterior and
submucosal tongue-tie can be done with a topical or local anesthesia.
Someone just has to hold the baby still...
