If everything seems to be good with the baby's intake, than maybe there is 
something going on with the baby's caloric usage or with baby's absorption?

Congenital heart defects can put increased demands on a baby's energy 
expenditure causing poor weight gain, or a malabsorption syndrom could 
prevent a baby from receiving adequate nutrition.
