I was asked to post this case (by the mom involved). We are hopeful that someone will have some input for her for future reference. Just to preface...she is a CLC and was working with 2 area IBCLCs, myself (a CLC), and her OB.

This mom pumped approx. 10 oz. every am from 1 1/2 months after birth in addition to nursing her baby. She had an extreme over supply. Several times she had tried to back off her pumpings/supply as she had a ton of milk but would fill up and be quite uncomfortable. She would pump for comfort (she was still nursing the baby as well). When baby was 11 months old she went away for 5 days (was separated from baby) and attempted to reduce supply. She pumped (out of being so full and uncomfortable) 3 x/day getting 10 oz. each time. She only pumped for comfort and never drained the breasts at this time. She felt over full all the time. There was never a time that she felt her body was absorbing. She just kept producing. No leaking. She then attempted to go 24 hours w/o any stimulation at all. She wound up after the 24 hours pumping (she was very uncomfortable at that point). 2 days later she tried to go 36 hours with same results, just more extreme. She then pumped and nursed the baby until she felt she "emptied" the breasts. The longest stretch she attempted was 5 1/2 days...which was unbearable for her and had to get some relief by pumping/nursing.

She had tried no stimulation, sage, cabbage leaves, Sudafed, birth control pills. Nothing reduced the amount of milk that she was producing. She would continue to fill up. Her prolactin and thyroid levels were drawn and were normal.

She finally was given 3 pills of Cabergoline (1/2 pill ea. dose...6 doses total). She seemed to finally get some relief using this method....1st dose she took, pumped 25 oz. and nursed her baby. 3 days later she filled up again, pumped and nursed. This process continued for a bit and is now completely weaned.

Any words of wisdom for this woman. Are there any conditions that would make a woman not able to naturally wean? Any suggestions as to where to look for answers?

Thanks so much in advance.
