Most gastric by-pass patients are taking in less than 1000 calories per day
the first year. When you get to under 1500 calories daily, milk production
can be affected. I would be very concerned for this mother's immediate
post-surgery breastfeeding ability because, if she follows standard
protocol, she will be losing weight at a tremendous rate by not eating much.
This simply does not adequately support lactation in the long run.

If has made up her mind to have the surgery, encourage her to keep
breastfeeding, but also to watch baby closely. Solids should be started by 7
mos so baby will have something else to provide calories, but still mother
needs to watch her own milk supply and be aware that baby may need
supplemental formula. Normally mothers further post-surgery (1-2 yrs or
more) require 70gs or more of protein daily plus vitamin supplements to
lactate well, especially keeping B-12 up.
