Hi All,

Mom's permission to post: 37-year-old, 2nd child (girl), 1st child (son, now 8)
bf for 1 year with no problems; induction at 41 weeks with C/S due to baby's
hand presenting first (son born without drugs, vaginally); 8-13 at birth, lowest
wt. 8-2; glucose water via bottle given in hospital, otherwise exclusively bf;
saw mom on day 7 at home with cracked painful L nipple, R side o.k.; as of
day 8 small crack on R side also; baby at 8-4 on day 6 and 8-4 still on day 10
at peds. office (ped. office not concerned); peeing/pooping well; cuing to
nurse well (mom scheduling feeds some to accommodate older child's needs);
cradle position needed some adjustment; baby's suck not consistently drawing
in breast properly- bunching up tongue some and what feels to be a more
pronounced clenching by baby's left upper gum; this clenching corresponds
with cracks which are at 12-3 on L side and 6-9 on R; using lanolin and XBM
and air drying for healing of nipples. L is painful to the extent that mom cannot
leave baby

I went over correct positioning/latch; suggested other nursing positions,
mentioned gel pads as an option, have encouraged feeding on demand instead
of the scheduled feeds, gave option of using pump/eyedropper feeds if L too
painful to nurse at this time (while still nursing from R), mentioned CST and
gave link for info.

Any thoughts- particularly with regard to the left upper jaw clamping?

Thank you in advance,
Dianne Oliver, IBCLC
