Saturday evening I received a call from a Mom who had been my patient in the
hospital 3 months ago. She called to discuss her baby and how it seemed not
to be satisfied for the last few days....constantly nursing. After
listening to how the baby was doing I was pleased to hear that this dear child
refuses a bottle. But that her pedi was thinking it needed rice cereal with formula
because a sibling had reflux and she thought this baby might be the same.
Mom's concerns went from did she have enough milk (has implants behind the
muscle) and she only pumped 3 oz. of EBM and maybe she didn't have enough. We
discussed and reviewed a lot of what she had heard in hospital and also what
she had read and what the pedi (which she said really didn't support
breastfeeding) and that the pedi Pulmono
logist told her to keep breastfeeding no matter what that it was the best
thing for all the allergies and reflux and everything. (Did I want to kiss that
man!!) I told her to keep his words foremost and to repeat them to the pedi
every time she heard the rice and formula solution...
Told her to keep pumping and freeze milk for when she wants a break and
teach someone to feed the baby by cup since nipples weren't in this kid's pattern
of feeding. Referred to the outside LC referral list and asked her to keep
in touch. We discussed using EBM to dilute cereal when the time is appropriate.
Now, did I contradict a MD ? Could be....but I really think this Mom
figured it out with a little help of her friends and they included a MD, RN IBCLC,
and the baby and a good dose of common sense...she tasted the cereal with
formula and said it made her gag...it was nasty stuff and that her milk was
sweet and nice. She credits her baby for showing her the way and just needed
reassurance.
So often we get the hard calls and sometimes we just need to realize we
don't contradict the desires of the Mom and baby. And in this case I say a well
read Pediatric Pulmonary doc trumps a non informed pediatrician anyday.!
